| Eligibility | Individuals who are eligible for admission to Campbell House are those suffering from a life-limiting illness with a prognosis of three months or less and are not seeking any curative treatment. Individuals must have involvement with their Home and Community Care Support Services.
Support and Programming is available to anyone who is living with a life-limiting illness, their caregiver(s) or anyone who is grieving a loss in the service area. 18 year(s) and up Residency Requirements: South Georgian Bay |
| Application | Referrals for Campbell House are made through the Home and Community Care Support Services and your Home Care Coordinator Referrals for caregiver(s), bereaved individuals seeking grief support/counselling or community members living with a life-limiting illness for support and programming can be made via self referrals or by other health care and support organizations.
